Group Dental Insurance


With the Delta Dental Plan the county pays the monthly premium for the single contract and the employee’s share of the family contract is $51.76. The benefit period is based on a fiscal year.

Check ups and teeth cleaning (diagnostic and preventive services) are paid at 100%, every six months, for each eligible member.

Deductible


$25 for single, or $25 per person with a maximum of $75 per family, deductible for routine and restorative services. A 80/20 coinsurance, or 50/50 coinsurance coverage for complex periodontal procedures and prosthetics, is offered. 

Out-of-Pocket Maximum


$1,000 for each eligible member per benefit period. Orthodontics paid at 50% up to a $2,000 lifetime maximum for unmarried, dependent children under age 23.

Group Health Insurance


Comprehensive Alliance Select Plan with Wellmark Blue Cross and Blue Shield, which is the preferred provider organization (PPO). Alliance Select preferred providers are subject to 90/10 coinsurance; nonselect providers incur a deductible, then are subject to 80/20 coinsurance.

American Federation of State, County and Municipal Employees (AFSCME) bargaining employees' portion of the monthly premium is currently $60 for the single contract and $160 for the family contract.

For management, attorneys, and confidential personnel, the employee portion of the monthly premium is currently $60 for the single contract and $160 for the family contract.

The benefit period is based on the calendar year. 

Deductible


The current single deductible is $300 and the family deductible is $600.

Out-of-Pocket Maximum


The current single out-of-pocket maximum is $1,000 and the family out-of-pocket maximum is $2,000.

VSP Vision Insurance


​VSP Vision Insurance is a voluntary benefit, and the employee pays the full premium. A single plan is $9.21 per month and a family plan is $19.80 per month.

For AFSCME Bargaining Unit and Conservation Employees


$15,000 of coverage and the premium is paid by the county, plus an option for the employee to purchase up to a maximum $100,000 additional coverage in increments of $5,000. Rates are age banded and subject to change each July 1 based on the employee's age.

For PPME Employees


$20,000 of coverage and the premium is paid by the county, plus an option for the employee to purchase up to a maximum $100,000 additional coverage in increments of $5,000. Rates are age banded and subject to change each July 1 based on the employee's age.

For Management, Attorneys and Confidential Personnel


$25,000 of coverage and the premium is paid by the county, plus an option for the employee to purchase up to a maximum $100,000 additional coverage in increments of $5,000. Rates are age banded and subject to change each July 1 based on the employee's age.

Group Long-Term Disability (LTD) Insurance


The county pays the monthly premium for a comprehensive LTD plan for all employees. If eligible, employee benefits include 66.66% of salary until age 65.
